programs designed to enhance the experiences of both patients and employees. We
understand that our caregivers can make a world of difference in the lives of
our patients and their families. We want our team to be as skilled and
motivated as possible in order to create a positive experience for each person
associated with our healthcare centers.
As a result, we have invested heavily in programs that promote quality care by
supporting staff. This means continuous educational opportunities and training,
management tools, and career development programs. Some of these programs are
highlighted below:
Acuity-Based Staffing (ABS):
An automated program that helps the healthcare center assign the appropriate
number and mix of professional nursing staff and aides. Based on the MDS and
RUGs, Acuity-Based Staffing looks at the acuity level of each patients and
produces a corresponding staffing recommendation of RNs, LPNs, and CNAs for the
unit.
TeamWorks:
A program designed to offer leadership training for Department Heads, assisting
with supervisory skill building, teamwork development, and performance
improvement.
Hospice of Fundamental relies on the combined knowledge of a highly
experience, diverse, empathetic team of professionals, which include:
CareWorks:
A career development program for Certified Nursing Assistants to increase skill
levels and provide opportunities for professional growth.
BRIDGE Mentoring:
A mentoring program designed to assist the newly hired CNA. This program
identifies accomplished CNAs to act as preceptors to new staff, assisting with
the period of adjustment to the facility.
QuickStart:
An orientation program developed for various facility employees including
Directors of Nursing, Nurse Assessment Coordinators, Charge Nurses,
Rehabilitation Staff, Dietary Professionals, Directors of Admissions, Staff
Development and Infection Control Nurses. The goal of this program is to help
new staff adjust to the expectations and routines of their position, and to
ensure they are aware of all resources available to them.

to make a positive difference in a patient's life. Family members often tell us
